Why Are My Teeth Suddenly Sensitive?

Teeth that were fine last month and now flinch at cold mean something changed. The usual suspects are gums that have crept back, a heavier grind at night, a recent whitening kit, or one tooth that needs a proper look.

So the useful move isn't grabbing a stronger toothpaste on day one. It's working out what changed in the last few weeks, because a couple of the possibilities deserve a phone call instead of a new tube.

I've practised in Bloor West Village for about 25 years, and this exact call comes in every week. "Nothing hurt before. Now ice water makes me wince. What gives?"

Most of the time the story is ordinary. Once in a while it's a tooth asking for help, and the whole point of this post is telling those two apart.

What's going on inside a suddenly sensitive tooth?

Tooth sensitivity. It's a short, sharp pain when something cold, sweet, sour, or even a breath of winter air touches a tooth. Under your enamel sits a softer layer called dentin, and dentin is threaded with thousands of microscopic fluid-filled tubes running toward the nerve.

When cold hits an exposed patch of dentin, the fluid in those tubes shifts, and the nerve reads that movement as a jolt. Researchers call this the hydrodynamic theory, and it's the leading explanation for sensitivity pain (BMC Oral Health).

It may not be the whole story. A 2021 review lists five proposed mechanisms and points out they aren't mutually exclusive, so more than one may be at work in the same tooth (J Endod review).

Here's why the plumbing matters to you. Enamel has no nerve supply, so a fully covered tooth feels nothing. If your teeth went from quiet to loud, some of those tubes got uncovered or opened up. Something did that.

Have your gums been quietly moving?

Are your teeth looking a little longer than they used to? Gum recession is the slowest, quietest way dentin gets exposed, and it's far more common than people think.

A 2025 meta-analysis pooling 22 studies estimated that about 81% of people have at least 1 mm of gum recession somewhere in the mouth (J Dent meta-analysis). Some recession is close to universal. The root surface it uncovers has no enamel over it, which is why a receded spot can suddenly announce itself one cold morning.

The Canadian trend line points the same way. Statistics Canada's direct oral health measurements found the share of adults whose gum pockets, the little groove between tooth and gum, measured a healthy 0 to 2 mm fell from 45% to 13% between 2007-2009 and 2022-2024, while 3 mm pockets rose from 34% to 51% and 4 mm pockets from 11% to 21% (Statistics Canada).

Did you know about this? Most of my patients haven't, and it means gum health is drifting the wrong way across the country, not just in your bathroom mirror.

The same meta-analysis found recession linked with smoking, alcohol, plaque, gum disease, and heavy bite forces, though these are associations rather than proven causes (J Dent meta-analysis). Could grinding be part of it?

Do you wake up with a tight jaw, or has someone mentioned noises from your side of the bed?

One of those associations deserves its own section: occlusal trauma, the clinical term for damage from heavy bite forces, including grinding and clenching, was linked with gum recession in that same analysis (J Dent meta-analysis). Heavy forces load the necks of the teeth, right where gum meets enamel, and that's often exactly where sensitive spots show up.

Grinding tends to ramp up in stressful stretches, which is one reason sensitivity can arrive out of nowhere. The habit changed before the teeth complained.

Did you whiten your teeth recently?

Most people think sensitivity after whitening means the kit damaged their enamel. Here's what's going on instead: peroxide gels temporarily open the pathway to the nerve, the teeth get louder for a while, and then they settle.

Health Canada's guidance on home whitening kits says it plainly: "Tooth sensitivity and soft tissue irritation may occur in some individuals, although these effects are transient and resolve on their own" (Health Canada).

The Canadian Dental Association's position statement matches: "When the products are used as directed, tooth sensitivity and soft-tissue irritation are the most common negative effects; these problems usually resolve on their own" (CDA position).

So if your sensitivity started days after you started a whitening kit, have no fear. The timing is your answer. Pause the kit, give it a week or two, and it should quiet down.

The place to be careful is duration. Health Canada notes that "Typical home-use tooth whitening products are used over a period of 10-14 days. For prolonged use, please make sure to consult an oral health professional" (Health Canada). Weeks of nonstop bleaching in pursuit of one more shade is where I see trouble. Are you scrubbing or sipping your enamel away?

Two habits I ask about every time, and this part is my plain advice from the chair rather than a research finding.

First, brushing pressure. A toothbrush is not a scrub brush. If your bristles splay out within a couple of months, or you brush with a fist grip and elbow behind it, you're likely wearing at the gumline. Try this: hold the brush like a pen, let soft bristles do the work, and give it two gentle minutes.

Second, acid. Frequent sips of pop, sports drinks, energy drinks, lemon water, or wine bathe your enamel in acid all day, and softened enamel plus enthusiastic brushing is a rough combination. I'd rather you finish a drink in one sitting than nurse it for three hours, and wait a while before brushing afterward. We wrote about what's in those cans in our post on energy drinks and teeth.

Neither habit announces itself. Both add up quietly until one cold drink tells you.

Does it hurt when you bite down, or when you let go?

This question sounds odd, and it's the one I most want you to notice.

Cracked tooth syndrome is a recognized condition, described in the literature as a series of symptoms caused by a cracked tooth, with its own body of work on how it gets diagnosed and managed (Pain Res Manag review). In practice, a hairline crack is easy to miss, even for the person whose tooth it is.

In 25 years of practice, one pattern I always test for is pain that spikes not when you bite down but at the moment you release. When a patient describes that, I get out a small bite stick and test each corner of the suspect tooth, one at a time, because that release-of-bite pattern is worth chasing in the chair.

To be clear, pain on releasing doesn't mean your tooth is cracked. It means the pattern deserves testing rather than a wait-and-see toothpaste. Cold sensitivity across several teeth and a sharp jolt from one specific tooth when chewing are two different animals, and only one of them should send you to the phone.

Which toothpaste should you buy tonight?

Here's the practical question, and for once the evidence is strong. A network meta-analysis pooled 125 randomized trials covering 12,541 patients to compare desensitising toothpaste ingredients head to head (J Dent Res meta-analysis).

The comparison is the interesting part. The winning ingredients didn't just beat a sugar-pill paste; they beat regular fluoride toothpaste (J Dent Res meta-analysis). Your everyday tube was the bar, and they cleared it.

Two ingredients came out best across the ways sensitivity gets triggered, touch, cold and air, with moderate to high certainty: calcium sodium phosphosilicate, a glass-like mineral compound that helps plug the open tubes, and stannous fluoride (J Dent Res meta-analysis). Potassium-based formulas, alone or combined with stannous fluoride or hydroxyapatite, also showed benefit (J Dent Res meta-analysis).

So flip the box over and read the ingredient list, not the marketing on the front. Any brand will do if the ingredient is right.

One honest caveat. This is symptom relief measured across groups of people, and it does nothing about whatever changed in your mouth. Quieter nerves are welcome; they just shouldn't end the investigation.

When is sudden sensitivity worth a call?

Pretty much all of this comes down to one sorting rule. Sensitivity that's mild, spread across several teeth, and traceable to a recent change can usually wait for a toothpaste trial and your next checkup.

A few patterns earn a call instead:

  • pain that lingers more than a few seconds after the cold is gone
  • pain that wakes you at night
  • swelling in the gum or face near a tooth
  • a sharp jolt when you release a bite
  • sensitivity pinned to one specific tooth rather than a general area
None of these mean disaster. Each one means a five-minute look is smarter than three weeks of guessing.
