
How Do You Actually Stop Grinding Your Teeth at Night?

Here's the honest answer up front: you mostly can't stop sleep grinding directly, because it happens while you're asleep and out of your control. A night guard, the fix most people picture, protects your teeth from the grinding rather than switching it off.
Nobody loves hearing that. It's still the right place to start, because half the advice online promises a cure that the research doesn't back up.

Is grinding your teeth even a disease?
Most people think grinding is a disorder that needs curing. Here's what the research says instead.
Bruxism (the clinical word for grinding and clenching) got a formal international rethink. A consensus group of researchers from around the world concluded that "in otherwise healthy individuals, bruxism should not be considered as a disorder, but rather as a behaviour" that can raise the risk of certain problems (international consensus).
That one sentence changes everything about the question in the title. You don't cure a behaviour the way you cure an infection. You manage what it does to your teeth and jaw, and you work on the things that feed it.
There's even a stranger wrinkle. A British overview for dentists notes "it has been postulated that sleep bruxism may have a protective role during sleep," for example around keeping the airway open or stimulating saliva flow (clinical overview).
Postulated means proposed, not proven, so hold that idea loosely.
The same overview is blunt about when to act: "Bruxism in itself does not require treatment: management is only indicated where problems arise as a result of bruxism" (clinical overview).
So if you grind a little and nothing hurts, nothing's cracking, and nobody's losing sleep? Have no fear. We watch it, and that can be the whole plan.
Why can't you just decide to stop?
Sore jaw at 7 am, headaches at your temples, a partner who can hear you from the other side of the bed. What gives?
Sleep grinding is run by the brain during sleep. The clinical overview describes it as a centrally controlled condition with a range of risk factors from around the body (clinical overview). You're not awake to catch yourself doing it, which is why willpower has nothing to grab onto.
Daytime clenching is a different animal. You can catch that one in the act: jaw tight at your desk, teeth touching while you concentrate. Teeth apart, lips together is the resting position, and noticing when you've drifted from it costs nothing.
The consensus group treats the two as separate things, defined separately (international consensus). Keep that split in mind, because it decides what the stress advice is worth.
In 25 years in Bloor West Village, I've usually spotted grinding before the patient has. Flattened edges on the canines, a filling that keeps chipping, teeth that have gone sensitive for no obvious reason. Doesn't stress cause night grinding?
Partly. Less than the internet says, and probably not for the night-time kind.
One study followed 387 dental students through the stressful years of their program, comparing stress and anxiety scores against grinding (dental student study). Stress tracked with daytime clenching, mostly in men during the highest-pressure years. Sleep grinding split along sex lines: it correlated with the emotional measures in the women only, and not in the men. The authors still concluded that sleep grinding "was not directly linked to emotional factors, implying a distinct etiology", meaning the two habits likely start in different places.
One study, self-reported grinding, weak correlations. I wouldn't build a treatment plan on it. It does fit the bigger picture, though: the "manage your stress and the grinding stops" promise is aimed at the wrong kind of bruxism.
Should you ignore stress, then? No. Relaxation and better sleep habits sit on the list of reasonable behavioural strategies in the clinical overview (clinical overview), and they cost nothing. Just don't expect them to switch off something your brain does while you sleep.
Is sleep apnea making you grind?
You'll find this claim everywhere: grinding means your airway is collapsing at night, and treating the apnea cures the grinding.
The evidence for adults doesn't support it.
One review of the question is literally titled "association, causality or spurious finding?" It looked at 13 adult studies and found no association between sleep apnea and sleep grinding in most of them (scoping review).
A second review, using full overnight sleep-lab recordings, reached the same wall: "There is no scientific evidence to support a conclusive relationship" between the two (systematic review).
Children are a different story, and an unfinished one. The same scoping review found the link seems plausible in kids, with insufficient evidence to confirm it either way (scoping review).
To be clear about what this doesn't mean: grinding your teeth is not a sign you have sleep apnea. If you snore loudly, if a partner has watched you stop breathing, or if you wake unrefreshed most mornings, those are reasons to raise sleep with your physician on their own, grinding or not.

Do coffee, alcohol and smoking make grinding worse?
They travel together, yes. Whether they drive it is a different question.
A review pooling seven studies found the odds of sleep grinding were about double in people who drank alcohol, more than double in current smokers, and about one and a half times higher in people drinking more than 8 cups of coffee a day (lifestyle review). The authors attach their own warning label: the findings stand "on the basis of limited evidence."
Read that caffeine number again. More than 8 cups a day. Your 2 pm americano is not the villain here, and dropping it is unlikely to change your nights.
Here's the other honest limit: these are associations. None of the supplied studies shows that cutting alcohol, caffeine or tobacco reduces grinding. People who grind drink and smoke more; that's not the same as proof that quitting quiets the jaw.
Pretty much everything on that list is worth moderating anyway, for reasons your whole body already knows. I'd just rather you did it with honest expectations than as a grinding cure.
Could a medication you're taking be involved?
Sometimes, and this section comes with the biggest caution flag in the post.
Researchers combed the World Health Organization's global side-effect database, where doctors and patients report suspected drug reactions, and found grinding reported alongside antidepressants far more often than expected (pharmacovigilance study). Of all the movement problems they checked, grinding showed the strongest signal.
Four things belong right next to that finding. It's a pattern of reports, not proven cause and effect. The authors themselves call the analysis exploratory. Nobody should stop or change a medication because of a blog post. If the timing fits, when the grinding started after a new prescription, the right move is a conversation with your prescriber, and we're glad to coordinate with them.
Antidepressants treat conditions that matter far more than a worn tooth edge does. The point isn't alarm. It's that your dentist should know your medication list, because sometimes the jaw is telling a story that started at the pharmacy.
What genuinely helps, then?
Now the useful part. A systematic review of sleep bruxism management, covering appliances, behavioural therapy, biofeedback and medication trials, gives the honest scoreboard (management review).
Night guards did more than protect: they tended to reduce the number of grinding episodes, though no one splint design beat the others. Protection is still their real job. Biofeedback, which trains your jaw using sensors, reduced grinding on specific measures. Cognitive behavioural therapy came out weaker: its potential benefits were not well supported.
A few prescription medicines also reduced grinding in small studies, with side effects reported alongside (management review). I'm deliberately not naming them. Small studies plus side effects makes that a physician conversation, not a shopping list.
Then there's botulinum toxin, injected into the jaw muscles. A review of 12 randomized trials found 6 reported reduced jaw muscle activity and 3 reported less muscle pain (botulinum toxin review). The clinical overview adds the counterweight: "concerns have been raised regarding possible adverse effects" (clinical overview). I'm reporting that evidence, not recommending the needle.
One thing to cross off your list entirely: permanently grinding down or reshaping teeth to "fix the bite" has no basis in evidence for treating bruxism (clinical overview). Don't let anyone remove healthy tooth structure chasing a habit that lives in the brain.
Some days my schedule runs behind because someone came in for a cleaning and left with a conversation about their jaw. That trade is always worth it. Catching grinding damage early is the difference between a smooth polish and, years later, a crown on a cracked tooth.
So here's the realistic plan. Protect the teeth with a guard. Catch daytime clenching in the act. Moderate the habits that travel with grinding. Review your medications with your prescriber if the timing fits. Then let us watch the wear, year over year.
